Koala-T Nomination Form

"Koala-T" is performance by an employee in the Division of Student Affairs wherein that person consistently chooses a positive attitude, even in the face of difficulties. This person displays amazing sensitivity to those he or she serves. The nominator should describe an instance (or many) in which the nominee did something that made a "Wow!" to someone he/she served or in something that the nominee did. The nominee should consistently display "Quality as a Value" in his/her service to others and the nominator should describe specific instances of such service.
